Constant awareness
Make your meditation a continuous state of mind. A great worship is going on all the time, so nothing should be neglected or excluded from your constant meditative awareness.
Actually, meditation is not about "living in some other frame". It is about being utterly in the moment right where we are. It is about accepting what is - not about escaping.
Of course that doesn't mean we shouldn't take action where action is appropriate. If you are feeling socially isolated I recommend that you get some help finding out why. A good therapist will be invaluable in this regard. Getting involved in an appropriate support group can also help.
